Acute Rehabilitation Case Manager – Simplified Job Details

Position: Acute Rehabilitation Case Manager
Start: ASAP
Shift: 8:00 AM – 4:30 PM
Schedule: Monday–Friday
Guaranteed Hours: 40/week

Requirements

• Case Management experience — required
• 2–3+ years preferred, but flexible based on experience
• Must have a current professional healthcare license as one of the following:
  • RN
  • Social Worker
  • Respiratory Therapist
  • Physical Therapist
  • Occupational Therapist
  • Speech-Language Pathologist
• Must have appropriate Case Management experience
• Acute hospital experience with adult patients is preferred
• Strong verbal and written communication
• Basic Excel, Word, and Outlook skills
• Good organization, time management, and analytical skills
• Ability to work independently and as part of a multidisciplinary team
• Must maintain patient confidentiality

Certifications

Case Management certification is preferred, not strictly required. Examples include:

• CCM – Commission for Case Manager Certification
• ARN – Rehabilitation Nursing certification
• ACM – Association of Rehabilitation Management
• ANCC RN-BC in Case Management

Facility & Unit

• 60-bed freestanding acute rehabilitation hospital
• Patient ratio: Approximately 1:15
• Patient population: Adults, including patients recovering from stroke, medical debilitation, and post-surgical conditions
• EHR: UDS PRO, MediTech, and Pinpoint
• Average rehabilitation length of stay: approximately 12 days

Key Responsibilities / Experience

The candidate should be comfortable with:

• Case management in an acute rehabilitation/IRF setting
• Utilization Review
• Supporting interdisciplinary team/family conferences
• Working with nursing, therapy, physicians, patients, and families
• Discharge/LOS planning and forward planning
• Balancing patient advocacy with appropriate use of healthcare resources
• Communicating professionally during difficult insurance or healthcare discussions

IRF team-conference experience is preferred, but the facility will provide assistance/training for this requirement.
