Job Summary:
 

The Physical Therapist will provide comprehensive physical therapy services to patients in an inpatient rehabilitation setting. The PT will evaluate patients, develop individualized treatment plans, provide therapy, collaborate with the interdisciplinary team, and facilitate safe and timely discharge.
 

Key Responsibilities:

• Complete comprehensive physical therapy evaluations for assigned patients.
• Use clinical reasoning to develop individualized treatment plans and measurable goals.
• Progress patients toward discharge within the established length of stay.
• Collaborate with the interdisciplinary team to identify and address barriers to discharge.
• Coordinate timely equipment trialing and ordering.
• Provide comprehensive patient and family training, including home exercise programs.
• Plan for appropriate continuing services following discharge.
• Manage individual, concurrent, and group therapy sessions based on patient needs.
• Provide coverage and demonstrate flexibility with caseload management.
• Ensure the plan of care is effectively communicated, implemented, and progressed throughout the patient's stay.
• Complete accurate and timely medical record documentation, including:
  • Daily documentation
  • Goal writing
  • Functional quality indicator assessments
  • Letters of medical necessity
  • Discharge documentation
  • Charge entry
• Ensure treatment is provided under valid physician orders and within applicable insurance authorization.
• Participate in student clinical education and provide appropriate supervision when assigned.
• Maintain an individualized professional development plan through continuing education and professional activities.
• Participate in patient and professional advocacy activities when applicable.
• Maintain professional licensure in good standing and comply with applicable State Practice Act requirements.
• Participate in quality and performance improvement initiatives.
• Maintain a safe environment for patients and staff, including safe patient handling, infection prevention, and workplace safety.

Required Qualifications:

• Minimum 2 years of Physical Therapy experience
• Valid, current State PT license
• BLS Certification required prior to first shift
• Strong clinical evaluation, treatment planning, documentation, and patient-care skills

Highly Desired Experience:

• Inpatient Rehabilitation experience
• Neuro rehabilitation experience
• Experience managing individual, concurrent, and group therapy sessions
• Experience with interdisciplinary discharge planning
• Patient/family education and home exercise program development

Schedule & Assignment:

• 8-hour shifts
• 5 days/week
• 40 hours/week
• Contract assignment up to 13 weeks
• 2 openings
• Inpatient rehabilitation setting
