HIGHLIGHTS:
- Active MI license or confirmation that application process has been initiated
- Your candidate understands OSF is a Drug-Free Campus and they must produce a negative drug screen for all panels
- Required Certifications: ACLS, BLS Certification
- Last four (4) digits of clinician's SSN
- Clinician's middle initial
- Requested time off
- License to Submit: Yes
- Shift Notes: 12hr Nights
ADDITIONAL DETAILS:
- Local Rate: Applies to clinicians with permanent addresses within 50 miles, accepted at a lower rate
- Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift
- Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation
- Cancellation Policy: 12 hours per 13 weeks
