Position Overview

The Pharmacist is responsible for providing safe and effective medication management services. The role involves reviewing and filling medication orders, monitoring patient medication therapies, providing drug-related information, identifying medication safety concerns, and supporting the pharmacy team through supervision and direction of support personnel.

Key Responsibilities

Medication Management

• Verify and accurately fill medication orders.
• Review medications to support safe and appropriate therapy.
• Monitor patients' medication therapies and identify potential concerns.
• Provide drug information to patients and healthcare professionals.
• Identify and troubleshoot potential medication safety hazards.
• Support safe and effective medication use throughout the course of treatment.

Documentation & Communication

• Document medication-related interactions and interventions for medical review.
• Maintain accurate and timely records related to medication therapy.

• Provide appropriate drug information based on patient and clinical needs.

Supervision & Team Support

• Supervise and direct pharmacy support personnel.

Required Qualifications

• Pharmacist – Required
• BLS – Required
• Ability to perform professional pharmacy duties related to medication verification, dispensing, and therapy monitoring.
• Ability to identify and address potential medication safety issues.
• Strong attention to detail and accurate documentation skills.
• Ability to communicate effectively with patients and healthcare professionals.
• Ability to supervise and direct pharmacy support personnel.
