Description

Job Description:
2 Years of acute-care Case Management experience.
Must have recent Care Coordination experience within the last 3 months.
Must have previous travel nursing experience.
Must have Epic experience.
Must have experience with Care Coordination and Discharge Planning.
Must be able to arrange DME, SNF and ambulance transportation.
Must be comfortable with a 1:25 patient ratio.
May be required to float to Laguna facility.
Weekend coverage is variable.
Case Managers do not perform Utilization Review.

Certifications/Requirements:
Active CA RN License required by start date.
Epic experience required.
Travel experience required.
