Job Summary: 

We are seeking an experienced Registered Nurse Case Manager to coordinate patient care, manage discharge planning, and ensure patients receive appropriate healthcare services. The RN will collaborate with physicians, nurses, and interdisciplinary teams to develop care plans, facilitate safe patient transitions, and support efficient hospital operations.

Responsibilities:

• Coordinate patient care and discharge planning in an acute care hospital setting.
• Assess patient needs and develop individualized care plans.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals.
• Evaluate medical necessity and conduct continued stay reviews.
• Coordinate post-discharge services, including home health, rehabilitation, and DME.
• Review admissions, authorizations, and insurance requirements.
• Assist with appeals, denials, and utilization management.
• Maintain accurate patient documentation using Epic.
• Ensure compliance with CMS regulations, HIPAA, and hospital policies.
• Float within Case Management services as needed.

Requirements:

• Active Alaska RN license required for Auto Offer.
• Minimum 1–2 years of RN Case Management experience.
• Recent acute care hospital experience required.
• Epic experience required (negotiable for the right candidate).
• BLS certification preferred.
• ACM certification preferred.
• Experience with discharge planning and care coordination.
• Knowledge of InterQual or Milliman criteria preferred.
• Must reside at least 50 miles from the facility.
• First-time travelers accepted.
