🚨 HARD-STOP REQUIREMENTS

Candidates must meet ALL of the following:

• 2 years of recent acute medical Care Manager experience
• Recent experience performing Care Coordination within the last 3 months
• Epic experience – REQUIRED
• Previous travel experience – REQUIRED
• Active California RN license required
  • Pending CA license may be accepted, but candidate must be licensed by start date
• First-time travelers NOT accepted

Position Overview

Providence Mission Hospital is seeking an experienced Registered Nurse Case Manager to support its Case Management department.

The Case Manager will focus on care coordination, patient assessment, discharge planning, treatment-plan development, and coordination of post-acute services.

⚠️ Important: This position is for Case Management only. Case Managers DO NOT perform Utilization Review (UR) in this role.

Key Responsibilities

• Assess patients and identify care coordination needs
• Develop and participate in patient treatment plans
• Coordinate discharge planning
• Arrange appropriate post-acute services
• Coordinate DME, SNF placement, and ambulance transportation
• Independently coordinate patient services and resources
• Participate in family meetings
• Work with HMOs, IPAs, and other healthcare organizations
• Support patients with complex social and medical needs
• Assist patients experiencing homelessness
• Coordinate resources related to mental health and substance abuse
• Understand advance directives and POLST processes
• Work with protective services when required
• Support adoption/foster-care related needs
• Maintain accurate computerized documentation
• Follow CMS, HIPAA, OSHA, Department of Health, and Joint Commission requirements

Unit Details

Department: Case Management
Patient Ratio: Approximately 1:25
Daily Census: Up to 25
Patient Population: Adults & Geriatrics
Orientation: 40 hours

Floating

• Floating is required to the Laguna facility.

Time-Off / Scheduling

• 5-day workweek
• Department operates 7 days/week
• Weekend coverage is variable
• Days off may vary and are not guaranteed to be back-to-back
• Up to 1 RTO request
• RTO restriction: No more than 7 days
