Travel Physical Therapist (PT) – Home Health
Augusta, Georgia
We are seeking a Travel Physical Therapist for a 16-week Home Health contract in Augusta, Georgia. This opportunity is a great fit for a PT who enjoys independent field-based care, one-on-one patient treatment, and helping adults and geriatric patients improve mobility, strength, safety, and functional independence in their homes.
This role offers a structured weekday schedule with a defined home health territory and a manageable daily visit expectation.

Responsibilities
- Perform home health physical therapy evaluations and reassessments.
- Develop individualized plans of care based on patient needs and functional goals.
- Treat patients with orthopedic, neurological, cardiopulmonary, post-surgical, and general mobility needs.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall-prevention education.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Complete OASIS documentation and maintain accurate home health records.
- Coordinate care with nursing, physicians, case management, and the interdisciplinary team.
Requirements
- Minimum 1 year of PT experience required.
- Georgia PT license or eligible Compact PT privilege required.
- BLS required.
- Home Health experience preferred.
- HCHB experience preferred.
- OASIS experience preferred.
- Valid driver’s license required.
- First-time travelers accepted.
