Description
Pride Health is hiring a Physical Therapist (PT) to support our client's healthcare facility in York, PA.
Job Details:
Schedule: Monday - Friday, 8:00 AM - 4:30 PM (40 hours/week)
Location: York, PA 17402
Float / Coverage Area: York County and Eastern Adams County
Job Type: Travel Contract (Local is also available)
Contract Length: 13 Weeks
Mileage Reimbursement Rate (if applicable): $0.50/ mile
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
- Complete Start of Care (SOC) visits, physical therapy evaluations, reassessments, and discharge visits for home health patients.
- Develop and implement individualized Plans of Care (POCs) to improve patient mobility, function, and independence.
- Manage an independent caseload and coordinate daily patient scheduling.
- Supervise Physical Therapist Assistants (PTAs) and ensure quality patient care delivery.
- Maintain accurate clinical documentation and participate in interdisciplinary care coordination.
Requirements:
- Education: Master's Degree in Physical Therapy or higher
- License(s): Active Physical Therapist (PT) License
- Certification(s): Physical Therapist Degree Required
- Years of experience: Home Health experience preferred
