Registered Nurse (RN) – Long-Term Care / Rehabilitation

We are seeking a compassionate and dedicated Registered Nurse (RN) to provide quality nursing care in a Long-Term Care and Rehabilitation setting.

Job Responsibilities

• Provide direct nursing care to residents in an LTC/Rehab environment.
• Assess residents, monitor vital signs, and recognize changes in condition.
• Administer medications and treatments as ordered.
• Develop and implement individualized resident care plans.
• Monitor residents’ progress and support rehabilitation and restorative care goals.
• Collaborate with physicians, therapists, CNAs, and other interdisciplinary team members.
• Provide wound care, pain management, fall prevention, and infection control as needed.
• Assist residents recovering from illness, injury, surgery, or hospitalization.
• Document assessments, care provided, medications, and resident progress accurately in the EMR.
• Supervise and guide CNAs and other nursing staff.
• Communicate effectively with residents, families, providers, and the healthcare team.
• Promote resident safety, dignity, independence, and quality of life.

Qualifications

• Current and unrestricted RN license in the applicable state.
• Previous LTC, skilled nursing, rehabilitation, or post-acute care experience preferred.
• Strong assessment, communication, documentation, and organizational skills.
• Ability to work effectively in a fast-paced, team-oriented environment.
