Contract Term: 13 Weeks
Schedule: Day Shift, 8:00 AM–4:30 PM
Guaranteed Hours: 24 Hours/Week
Facility: Samaritan Albany General Hospital
EMR: EPIC

Requirements:
Active Oregon LMSW or CSWA licensure required at submission
Oregon LCSW preferred
Healthcare Provider Level BLS certification required
Minimum 1 year of experience required
Previous hospice experience preferred
Experience as a social worker in a hospital or medical setting preferred
Valid driver's license required
Personal automobile required for the position
Valid auto liability/property damage insurance as required by law
Ability to maintain personal vehicle in good working order
Strong understanding of medical social work and hospice services
Ability to support patients and families through end-of-life care
Ability to work independently in home-based care settings

Responsibilities:
Provide medical social work services to hospice patients and their families
Support patients and families through the final stages of illness
Provide services in patients' homes, skilled nursing facilities, assisted living facilities, and adult foster homes
Provide counseling and psychosocial support to patients and family members
Assess patient and family needs and coordinate appropriate resources
Collaborate with hospice nurses, physicians, aides, volunteers, spiritual support staff, and other members of the care team
Assist with pain and symptom management support as part of the interdisciplinary hospice team
Provide bereavement and end-of-life support as appropriate
Help patients and families navigate available healthcare and community resources
Maintain appropriate documentation in the EPIC medical record
Protect patient privacy and confidentiality
Provide services across assigned communities within the hospice service area
Use personal automobile for required patient visits
Follow facility policies, professional standards, and applicable regulations
Participate in holiday coverage as required
Provide other duties as assigned

Schedule: Day Shift 8:00 AM–4:30 PM No Regular Weekends 24 Hours Per Week 13-Week Contract
