Seeking an experienced Ambulatory RN/OB Case Manager to provide comprehensive, patient-centered nursing care in an outpatient OB clinic. The RN will coordinate care, manage referrals and follow-ups, educate patients and families, collaborate with OB providers and interdisciplinary teams, and help ensure continuity of care throughout the patient's treatment journey.

Requirements:

• Active, unrestricted Alaska RN license required at time of submission
• OB Case Management experience required
• BLS required
• Case Management certification preferred
• Graduate of an accredited nursing program
• BSN preferred

Key Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans
• Coordinate OB patient care, referrals and follow-up services
• Collaborate with providers and interdisciplinary teams
• Provide patient and family education
• Monitor patient needs and escalate changes in condition appropriately
• Assist patients with navigating healthcare services
• Administer medications and perform clinic-based nursing procedures as ordered
• Maintain accurate and timely EHR documentation
• Promote patient safety, quality care and culturally responsive services
• Participate in required training, competencies and quality-improvement activities
