Description

Pride Health is actively hiring a Registered Nurse (RN) - Home Health Case Manager to join our client's Home Health Team.

 

Location: Uniontown, OH 44685

Patient Care Setting: Home Health
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Hours: 40 Hours Per Week (36 Hours Guaranteed)
Productivity Expectation: 30 Points
On-Call Requirement: 1 Week Per Rotation
Weekend Requirement: 1 Weekend Rotation
Coverage Areas: Summit, Portage, and Medina Counties, Ohio
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Navy Blue or Black Scrubs

Position Overview

  • We are seeking an experienced and compassionate Home Health Registered Nurse (RN) Case Manager to provide skilled nursing services and comprehensive case management to patients throughout Summit, Portage, and Medina Counties.
  • The ideal candidate will have strong clinical assessment skills, home health experience, and the ability to independently manage a patient caseload while delivering exceptional patient-centered care.

Key Responsibilities

  • Manage a home health caseload and serve as the primary coordinator of patient care services.
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and routine skilled nursing visits.
  • Conduct comprehensive patient assessments and develop individualized plans of care in collaboration with physicians and interdisciplinary team members.
  • Monitor patient conditions, evaluate progress toward goals, and communicate significant changes to physicians and care teams.
  • Provide skilled nursing care, medication management, disease management education, and patient/caregiver training.
  • Coordinate services with therapists, physicians, social workers, caregivers, and community resources to ensure continuity of care.
  • Complete accurate and timely clinical documentation in compliance with agency, state, and federal regulations.
  • Meet productivity expectations while maintaining high standards of patient care and customer service.
  • Participate in rotating on-call and weekend schedules as assigned.

Required Qualifications

  • Active Ohio Registered Nurse (RN) License.
  • Home Health nursing experience.
  • Strong assessment, care planning, and case management skills.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to work independently in a field-based environment.
