Description
LPN – Long-Term Care (LTC)
Location: Albuquerque, NM
Job Summary
Seeking an experienced LPN to provide safe, compassionate nursing care to residents in a long-term care/skilled nursing facility. The LPN will work closely with RNs, CNAs, physicians, residents, and families to ensure quality patient care.
Responsibilities
- Administer medications and treatments as ordered.
- Monitor residents and report changes in condition promptly.
- Provide wound care and other nursing procedures within scope of practice.
- Assist with ADLs, mobility, comfort, and resident safety.
- Follow individualized care plans and facility protocols.
- Complete accurate nursing documentation and medication records.
- Communicate effectively with the healthcare team and provide shift reports.
- Follow infection-control and patient-safety guidelines.
Requirements
- Active New Mexico LPN license.
- LTC/SNF experience preferred.
- Current BLS/CPR certification.
Strong clinical, communication, and documentation skills.
