Travel Physical Therapist – SNF/Rehab
Holyoke, MA | Western Massachusetts
Ardor Health Solutions is seeking a Physical Therapist for a travel Skilled Nursing/Rehab contract in Holyoke, Massachusetts.
Position Overview
This position works primarily with an adult and geriatric rehabilitation population, helping patients regain strength, mobility, balance and functional independence following hospitalization, surgery, illness or injury.
Responsibilities may include:
- PT evaluations and reassessments
- Development and progression of individualized plans of care
- Therapeutic exercise and strengthening
- Gait and balance training
- Transfer and functional mobility training
- Fall prevention and safety education
- Assistive device training
- Patient and caregiver education
- Collaboration with OT, SLP, nursing and the interdisciplinary rehab team
- Discharge planning
