Travel Physical Therapist – SNF/Rehab
Wilmington, MA | Merrimack Valley
Contract: October 5, 2026 – January 4, 2027
Ardor Health Solutions is seeking a Physical Therapist for a travel Skilled Nursing/Rehab contract in Wilmington, Massachusetts.
Work with an adult/geriatric rehabilitation population helping patients regain mobility, strength, balance and functional independence following hospitalization, surgery, illness or injury.
Responsibilities
- PT evaluations and reassessments
- Develop and progress individualized plans of care
- Therapeutic exercise and strengthening
- Gait, balance and transfer training
- Functional mobility and fall prevention
- Assistive device training
- Patient and caregiver education
- Collaborate with nursing, OT, SLP and the interdisciplinary rehab team
- Support discharge planning and safe transitions
