Ardor Health Solutions is seeking a Physical Therapist for a travel Home Health contract with CarePartners Home Health in Asheville, North Carolina.
Position Overview
This is a Home Health PT opportunity providing skilled therapy to patients in their homes throughout the Asheville area. Home Health gives therapists more autonomy and the opportunity to provide individualized, one-on-one patient care while helping patients safely regain mobility and independence in their own environment.
Responsibilities
- Complete initial PT evaluations and reassessments
- Develop individualized plans of care
- Provide one-on-one therapy in patients’ homes
- Gait, balance and transfer training
- Therapeutic exercise and functional strengthening
- Fall prevention and home-safety education
- Assess assistive device and equipment needs
- Educate patients and caregivers
- Coordinate care with nursing, physicians and other members of the Home Health team
- Complete required clinical documentation
- Assist patients in maximizing safety and independence at home
