Required Qualifications

• Minimum 2 years of Acute Care Case Management / Discharge Planning experience.
• Hospital experience is mandatory. Behavioral health experience alone will NOT qualify.
• Previous travel nursing experience required. First-time travelers are NOT accepted.
• Active California RN license required by the start date. Pending license accepted if obtained before starting.
• Experience with patient assessment, care coordination, discharge planning, and utilization review.
• Ability to independently arrange DME, SNF placement, and ambulance transportation.
• Experience with HMOs, IPAs, prior authorizations, and interdisciplinary care coordination.
• Strong knowledge of hospital regulations, CMS guidelines, and discharge planning processes.
• Proficiency in computerized documentation.

Key Responsibilities

• Assess patients and coordinate appropriate care plans.
• Manage discharge planning and utilization review.
• Coordinate with physicians, families, social services, and physical therapy.
• Organize and participate in family meetings.
• Arrange DME, skilled nursing facility (SNF) placements, and ambulance transportation independently.
• Support patients with complex social needs, including homelessness, substance abuse, mental health concerns, and advance directives.
• Ensure compliance with CMS, The Joint Commission, and other regulatory requirements.

Additional Details

• Patient Ratio: Days 1:25; weekends may reach 1:45.
• Weekend Requirement: Every other weekend, based on departmental needs.
• Floating: Not required.
• Orientation: 40 hours.
• Time Off: Maximum 7 days of requested time off.
• Dress Code: Business attire or scrubs.
• COVID-19 Vaccine: Not required.
