Description
RN Case Manager – IRF 🏥
- M–F, 8a–5p — 5x8s
- 40 hrs/week
- Start ASAP or 10/19/26
- 2+ years acute care or Inpatient Rehab Case Management / Utilization Review
- TX RN license + BLS required
- WellSky EMR
- Interview required
- Must work between Dallas & Houston sites
- PRNs heavily used for weekends/after-hours admissions
- Avg. LOS ~11.2 days — quick turnaround
- Responsible for discharge planning, continued-stay reviews, DME, HH, outpatient therapy & dialysis coordination
- Initial discharge plan due within 2 days of admission
