Openings: 1
Position Overview:
Chenega Northern Cheyenne Service Unit is seeking a qualified Diabetes Pharmacist to provide comprehensive pharmacy and diabetes-focused clinical services. The pharmacist will support medication management, diabetes care, patient education, and collaborative clinical services while working closely with providers and other healthcare professionals.
Key Responsibilities:
- Provide comprehensive pharmacy services and medication management to patients.
- Support the management of patients with diabetes and other chronic conditions.
- Participate in collaborative practice and work closely with physicians and other healthcare providers.
- Review medication regimens for appropriateness, safety, effectiveness, and potential interactions.
- Provide diabetes-focused medication counseling and patient education.
- Support the use and management of continuous glucose monitoring (CGM) technology.
- Assist patients with medication adherence, disease management, and lifestyle-related education.
- Monitor clinical outcomes and make appropriate medication-related recommendations.
- Provide immunization services within applicable scope of practice.
- Maintain accurate and timely documentation of clinical interventions and patient care.
- Communicate effectively with patients, providers, nursing staff, and other members of the healthcare team.
- Follow applicable pharmacy policies, clinical protocols, regulatory requirements, and patient-safety standards.
- Maintain professional standards, confidentiality, and appropriate documentation.
Required Professional Qualifications:
- Graduate of an accredited College of Pharmacy.
- Current unrestricted pharmacist license valid in any U.S. state, Washington, D.C., Puerto Rico, or U.S. territory.
- Current documentation of required Continuing Pharmacy Education (CPE) for licensure.
- Immunization certification required.
- Board Certification in Ambulatory Care Pharmacy (BCACP) or Board Certification in Pharmacotherapy (BCPS), OR completion of a PGY1 Pharmacy Residency.
- Demonstrated experience with diabetes management and collaborative practice agreements.
- Demonstrated proficiency with Continuous Glucose Monitoring (CGM).
- Fluent in English.
- Must maintain professional hygiene and grooming standards.
- Contractor must provide a resume documenting previous professional experience.
Work Schedule:
Pharmacy Department schedules consist of 8-hour shifts.
Monday – Friday
Available shift windows include:
- 7:00 AM – 4:00 PM
- 9:00 AM – 6:00 PM
- 10:00 AM – 7:00 PM
Weekend
At select facilities:
- 8:00 AM – 5:00 PM
Federal holidays are not worked.
