Job Title: Master of Social Work (MSW)
Location: Mission Viejo, CA
Job Type: 13-Week Contract
Start Date: 11/02/2026
Shift: Days | 8:00 AM – 4:30 PM
Schedule: 5 days/week | Sunday–Thursday
Position Overview:
We are seeking an experienced Medical Social Worker (MSW/LCSW) for an acute care hospital setting. This role focuses on case management, discharge planning, care coordination, and post-acute care needs for adult and geriatric patients.
The Care Management team includes RNs and MSW/LCSWs working collaboratively to ensure safe, timely, and cost-effective patient discharges.
Manage a caseload of approximately 20–23 patients
Document assessments, care plans, interventions, and patient responses in EPIC
Required Qualifications:
MSW required
Minimum 1 year of acute care hospital experience required
Recent hospital experience required
EPIC experience
Previous travel experience required – NO first-time travelers
Experience in medical social work, case management, and discharge planning
BLS required
LCSW preferred
