Contract: 13 Weeks
Shift: Days | 5×8 | 8:00 AM–4:30 PM

EMR: Epic — HARDSTOP

First-Time Travelers: Not accepted — Travel experience is a HARDSTOP

Requirements

Minimum 2 years of recent acute medical Care Manager experience
Recent Care Coordination experience within the last 3 months
Epic experience required
Active CA RN License required by start
Previous travel RN experience required
Strong experience with:
- Care Coordination
- Discharge Planning
- Patient Assessments
- Treatment Planning
- Family Meetings
- DME, SNF & ambulance arrangements
- HMO/IPA coordination
- Complex psychosocial/discharge needs
Must be able to independently arrange DME, SNF placement and transportation
Trauma Level I/II experience preferred
