REQUIRED EXPERIENCE

• Minimum 2 years of Home Health (HH) experience with high-volume caseloads — no exceptions.
• Recent OASIS experience required, including familiarity with OASIS E11 and OASIS-D charting.
• Recent travel experience required; first-time travelers are not accepted.
• Strong assessment skills, especially for heart failure patients.
• Required experience with:
  • Start of Care (SOC), Resumption of Care (ROC), admission assessments, and routine visits
  • High-volume wound care, wound staging, and Wound VAC management
  • PICC line dressing changes
  • IV therapy, peripheral IVs, central lines, and ports
  • Medication assessment and management, anticoagulant management, and blood glucose monitoring
  • Urinary and suprapubic catheter care
  • Feeding tubes, ostomy care, drains, oxygen, and nebulizer teaching
  • Infection control, pain assessment, pressure ulcer prevention, and clinical documentation
• Must be able to complete daily patient visits within the scheduled 8-hour shift.
• Must have a current driver’s license.

SCHEDULE

• Day shift: 8:00 AM–4:30 PM, 5×8 hours.

REQUIRED CERTIFICATIONS

• Active BLS required.
• Recent OASIS experience required; candidates must take an OASIS exam to qualify for an interview.
• Review OASIS E11 before the exam. Mobility topics include the G section, GG section, and M18.
• Flu vaccine required; no exemptions.
• COVID-19 vaccine: Not required.

LICENSE

• Active California license required at submission.
• Pending or temporary licenses are not accepted.
• Current driver’s license required.
