Description
Registered Nurse (RN) – Case Manager
Location: West Hollywood, CA
Engagement Type: Travel Contract
Contract Duration: 13 Weeks
Contract Start Date: November 2, 2026
Shift: Days
Shift Time: 8:00 AM – 5:00 PM
Schedule: 5x8-hour shifts
Weekend: Every Other Weekend (per unit details)
Key Responsibilities
- Coordinate patient care and collaborate with physicians, nurses, social workers, and other healthcare professionals.
- Perform case management and utilization review activities to ensure appropriate and cost-effective patient care.
- Develop and implement individualized discharge plans based on patients' clinical needs and available resources.
- Assess patient needs and coordinate appropriate post-acute care services and follow-up arrangements.
- Review medical records and clinical documentation to support medical necessity and appropriate resource utilization.
- Utilize InterQual criteria and Milliman Guidelines for utilization review, as applicable.
- Monitor patient progress and facilitate timely transitions of care.
Requirements
- Minimum 2 years of acute care case management experience.
- Previous California case management experience required; 6 months of California case management experience is acceptable according to the job overview.
- Active, unrestricted California Registered Nurse (RN) license in hand at the time of submission.
- Current BLS certification.
- EPIC experience highly preferred.
