Description
Position: RN Case Manager
Shift: 8-hour days, 08:00 AM–04:30 PM
Hours: 40 hours per week
Experience: Minimum 1.5 years of general nursing experience; first-time travelers accepted with more than 2 years of experience
Required certification: BLS
Required charting system: EPIC experience required
Education: Completion of a board-approved nursing education program and RN eligibility/licensure through the South Dakota Board of Nursing
Patient ratio: 1 case manager to 16–20 patients
Orientation: 1.5 weeks, including EPIC training
Key responsibilities and skills
- Acute care case management and discharge planning
- Progression-of-care coordination
- Meeting with patients and families to discuss plans of care
- Coordinating care across acute care departments
