NOW HIRING: MSW – MEDICAL SOCIAL WORKER / CASE MANAGEMENT

📍 Facility: Providence Mission Hospital
📌 Location: 27700 Medical Center Road, Mission Viejo, CA 92691
📅 Start Date: November 2, 2026
📆 Contract: 13 Weeks
🕒 Shift: Day Shift | 8:00 AM – 4:30 PM
📅 Schedule: 5 days/week, Sunday–Thursday
👥 Caseload: Approximately 20–23 patients
🏥 Unit: Acute Care Inpatient – Case Management
🎨 Scrubs: Turquoise
 

Required Qualifications

• MSW (Master of Social Work)
• Minimum 2 years of experience per job requirements
• At least 1 year of acute-care inpatient hospital experience
• Recent hospital experience within the last 3 months
• Previous travel experience required — first-time travelers are not accepted
• Epic charting experience required
• BLS certification

Key Responsibilities

• Hospital case management and discharge planning
• Initial biopsychosocial assessment within 24 hours
• Coordinate safe patient discharge and post-acute care
• Arrange home health, DME, rehabilitation, and long-term care services
• Coordinate with physicians, insurance case managers, and the care team
• Patient and family education, referrals, and resource coordination
• Document assessments, care plans, and discharge arrangements in Epic

Important Candidate Restrictions

• Behavioral Health-only or Emergency Department-only social work experience is not accepted.
• Candidates must have acute-care inpatient hospital experience.
• Schedule may change based on staffing needs.
• Floating to a nearby Laguna facility may be required.
• California license is not required per the job details.
• Local candidates within 50 miles may be accepted at a lower bill rate.
• Maximum 7 days of requested time off (RTO); disclose all requests before submission.
• Must be available to work 3 of the 5 designated holidays.
• Only one Providence facility submission at a time.
