Travel Occupational Therapist (OT) – Skilled Nursing Facility
Lake City, Florida
We are seeking a Travel Occupational Therapist for a 13-week Skilled Nursing Facility contract in Lake City, Florida. This opportunity is a great fit for an OT with SNF experience who enjoys post-acute rehab, geriatric care, and helping patients improve independence, safety, ADLs, and overall functional ability.

Responsibilities
- Evaluate patients’ functional abilities and develop individualized treatment plans.
- Provide skilled occupational therapy services in accordance with physician orders and facility protocols.
- Address ADLs, upper-extremity function, cognition, safety, mobility, and functional independence.
- Educate patients and caregivers on adaptive equipment, compensatory strategies, and home programs.
- Collaborate with nursing staff, PT, SLP, and the interdisciplinary rehab team.
- Complete accurate and timely documentation and progress reporting.
Requirements
- Florida OT license required.
- SNF experience required.
- Time-off requests must be submitted with the original submission.
- 30-day notice policy applies.
