Home Health Registered Nurse (RN) – Days

Job Description

We are seeking an experienced Registered Nurse for a 26-week Home Health assignment in Barre, Vermont.

The RN will provide individualized, goal-oriented nursing care to patients in their homes in accordance with physician orders and established nursing standards. Responsibilities include patient assessment, care planning, implementation and evaluation of nursing interventions, clinical documentation, and coordination of care. The RN will also serve as a case manager when acting as the lead discipline.

Qualifications

Minimum 1 year of nursing experience
Current, active nursing license appropriate for the assignment
Current BLS/BCLS certification
Home health nursing, patient assessment, and care coordination experience preferred

Schedule and Assignment Details

Shift: Days
Schedule: Monday–Friday
Guaranteed hours: 36 per week
Number of Contract Weeks Duration: 26 weeks
EMR: Homecare Homebase (HCHB)
Holiday coverage: Not required
Mileage reimbursement: Available for travel between patients at the applicable IRS rate; commuting mileage is excluded
