Home Health RN – Case Manager

Location: St. Albans, Vermont Shift: Monday–Friday, 8:00 AM–4:00 PM   Specialty: Home Health RN / Case Manager

Position Details

• Provide home health nursing care and manage assigned patient caseloads.
• Complete OASIS assessments and documentation accurately and on time.
• Develop, implement, and evaluate individualized plans of care.
• Conduct patient visits, coordinate care, and supervise LPNs and home health aides.
• Provide patient and caregiver education.
• Perform IV care, wound VAC management, and complex wound care.
• Coordinate with physicians, therapists, social workers, and other care team members.
• Maintain accurate documentation using McKesson processes.
• Coordinate patient discharges, transfers, case conferences, and quality audits.

Requirements

• RN license eligible to practice in Vermont.
• Home health nursing and case management experience.
• OASIS documentation experience.
• Experience with IV therapy, wound VACs, and complex wound care.
• Strong care coordination, patient education, and documentation skills.
• Ability to manage a patient caseload and conduct home visits.
