Description

Job Description

The Home Health LPN will provide and document nursing care for patients in their homes, supporting recovery, rehabilitation, health maintenance, and illness prevention. The nurse will work independently under general supervision while collaborating with healthcare professionals, patients, families, and community representatives to coordinate effective home care.

Responsibilities

Provide and document nursing care for patients in home health settings.
Monitor patient conditions and evaluate responses to nursing interventions.
Coordinate care with healthcare providers, patients, families, and community representatives.
Apply community health principles and nursing practices.
Maintain accurate patient records and communicate clinical information effectively.
Demonstrate critical thinking, independent judgment, and effective time management.
Follow nursing protocols, patient rights, confidentiality requirements, and facility policies.
Participate in flexible scheduling and accept holiday and weekend assignments as required.

Required Qualifications

Active Massachusetts LPN license.
Minimum 1 year of acute nursing care experience; hospital-based experience preferred.
