RN Case Manager
Must have 2 years of Case Manager experience required
2-year acute Hospital Case Management experience performing Care Coordination and discharge planning
First-timers accepted: Yes as a SCREEN with Clinical’s approval
REQUIRED:
* Experience with HMOs, IPAs, etc.
*Experience with advance directive, POLST, protective services process
HARDSTOP- ACUTE CARE Hospital Case Management EXPERIENCE AND MUST BE CURRENT WITHIN THE LAST YEAR.
Epic experience required.
