Travel Occupational Therapist (OT) – Skilled Nursing Facility
Concord, Massachusetts
We are seeking a Travel Occupational Therapist for a 13-week Skilled Nursing Facility contract in Concord, Massachusetts. This opportunity is a great fit for an OT who enjoys post-acute rehabilitation and helping adult and geriatric patients improve independence with daily activities, mobility, strength, cognition, and overall function.
The position offers 36 guaranteed hours per week, with scheduling typically around 40 hours per week.

Responsibilities
- Perform occupational therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient needs and functional goals.
- Address ADLs, upper-extremity function, coordination, cognition, safety, and functional mobility.
- Provide therapeutic activities designed to improve independence and quality of life.
- Educate patients and caregivers on adaptive equipment, compensatory strategies, and home programs.
- Collaborate with nursing staff, PT, SLP, and the interdisciplinary rehab team.
- Complete accurate and timely documentation and progress reporting.
Requirements
- Massachusetts OT license required.
- BLS required.
- SNF or post-acute rehab experience preferred.
- Strong communication and interdisciplinary teamwork skills preferred.
