Job Title: RN Case Manager
Location: Oklahoma City, OK 73104
Work Shift: Days
Number of Positions: 1
Hours: 36 hours per week | 3 × 12-hour shifts
Job Summary:
The RN Case Manager is responsible for coordinating patient care and services as part of a multidisciplinary care management team.
This role involves assessing patient needs, monitoring clinical progress, facilitating discharge planning, and coordinating resources to ensure continuity of care and appropriate utilization of healthcare services.
Conduct comprehensive patient assessments, including clinical, psychosocial, environmental, discharge planning, and financial needs.
Required Skills and Experience:
- Minimum of 3 years of clinical nursing experience in an acute care facility.
Education Requirements:
- Graduate of an accredited school of nursing.
Required Certifications and Licensure:
- Current Registered Nurse (RN) license issued by the Oklahoma State Board of Nursing, or an active multistate Enhanced Nurse Licensure Compact (eNLC) license.
- Current Basic Life Support (BLS) certification issued by the American Heart Association (AHA), required upon hire.
