Description
Job Title: Registered Nurse (RN) – Case Manager
Location: Brooklyn, NY 11212
Duration: 13 Weeks
Start Date: January 2, 2027
Schedule: Monday–Friday
Shift: 7.5 hours/day
Hours: 37.5 hours/week
Position Overview:
Seeking an experienced RN Case Manager with a strong clinical background to coordinate patient care, advocate for safe and timely discharge planning, and ensure medical necessity and compliance with healthcare regulations.
Required Qualifications:
- Minimum 3 years of experience in one or more of the following:
- Acute Care
- Medical-Surgical
- Intensive Care Unit (ICU)
- Emergency Department (ED)
- Community Nursing / Home Care
- PRI certification/proficiency.
- Strong clinical assessment, care coordination, and discharge planning experience.
- Knowledge of Milliman Care Guidelines and CMS guidelines.
Education:
- Bachelor of Science in Nursing (BSN) – Required.
Required Licensure & Certifications:
- Active New York State RN License.
- BLS.
- ACLS.
- Certified RN Case Manager.
