Coordinates patient care through collaboration with the Interdisciplinary Care Transitions (ICT) team.
Follows patients across the continuum of care, with care planning, plan implementation, and ongoing evaluation.
Integrates case management, utilization review and management, and discharge planning to promote continuity of care and cost effectiveness.
Ensures optimal use of resources, service delivery, and compliance with external review agencies.
