Position Overview

We are seeking an experienced Registered Nurse (RN) specializing in Case Management for a travel assignment at Mission Hospital in Asheville, North Carolina. This position offers a consistent daytime schedule, a collaborative healthcare environment, and the opportunity to work at an 815-bed hospital serving patients across multiple specialties.

The ideal candidate will have strong clinical judgment, excellent documentation and communication skills, and experience coordinating patient care and discharge planning across multidisciplinary teams.

Key Responsibilities

• Coordinate patient care and discharge planning across multiple hospital units.
• Collaborate closely with RN Case Managers, physicians, social workers, nursing staff, and other healthcare professionals.
• Assist with identifying patient needs, discharge barriers, and appropriate post-acute care resources.
• Facilitate communication between patients, families, and multidisciplinary care teams.
• Maintain accurate and timely documentation within hospital electronic medical record systems.
• Support safe and efficient patient transitions between levels of care.
• Manage multiple patient cases while prioritizing tasks in a fast-paced hospital environment.
• Build trusting relationships with patients, families, and healthcare team members.
• Assist with coordinating post-acute services and ensuring continuity of care.

Qualifications

Required:

• Active Registered Nurse (RN) license with authorization to practice in North Carolina.
• Previous hospital-based RN Case Management experience.
• Strong clinical documentation and organizational skills.
• Excellent verbal and written communication abilities.
• Ability to manage competing priorities and shift attention between tasks.
• Demonstrated ability to collaborate with multidisciplinary healthcare teams.
• Strong interpersonal skills and ability to establish trust with patients and families.

Preferred:

• Previous travel nursing experience.
• Experience using Cerner, NATE, or Ensocare.
• Experience coordinating discharge planning and post-acute care services.
• Experience in a large acute-care hospital environment.
