Overview
- Full-time home health PT in Ohio
- Covers Dayton, Huber Heights, Fairborn, and Yellow Springs territories
- Travel/Contract assignment
- 13-week contract
- Day shift, 5x8 hours, 8:00 AM to 5:00 PM
- 40 hours per week
Required Qualifications
- Valid Physical Therapist license in Ohio
- BLS
- Driver’s License required for all candidates
- State license verification required
- Background check required
- State fingerprinting required
- Negative TB or Chest X-Ray required before start
- Hepatitis B documentation required
- Personal or auto insurance required
- Photo for badge required
- Skills checklist required
- Competency exam required as applicable to profession/specialty
- Medication test/pharmacology competency exam required as applicable to profession
- Orientation Safety Manual acknowledgement required for clinical candidates
- Physical exam required for candidates working in Kentucky only
- Drug testing required for Home Health/Hospice candidates working in Indiana only
