Job Description
- Required Certification: EKG Certification

Background Checks and Screening Requirements
- 7-year background check including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and international search if applicable
- FACIS Level 3 screening within 2 weeks of start and monthly thereafter (or equivalent checks if FACIS III not available)
- EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List, Medi-Cal Suspended and Ineligible Provider List (as applicable)
- Sex Offender Search within 90 days of start
- Education verification (not required if highest education is high school)
- Transcript/Diploma verification if education verification unsuccessful
- Employment verification for up to 7 years with explanation of gaps over 30 days
- References: 2 professional references required for clinical/allied/per diem acute care candidates (Aya requirement only)
Health and Immunization Requirements
- Physical Exam within 1 year of start (must indicate cleared for work)
- Fit Mask Test within 1 year of start, renewed annually (exceptions apply)

- Negative TB test or chest X-ray within 90 days, renewed annually
