Requirements

• Licensure: NY LPN license in hand
• Experience: Hospital experience within the last 2 years
• Ability: Must be able to take own patient assignment with RN support
• Prior Travel Experience: Not explicitly required

Background and Compliance Checks

• 7-year criminal background check (felony checks) within 3 months of start
• EPLS/GSA/SAM check within 3 months of start and monthly thereafter
• HHS/OIG check within 3 months of start and monthly thereafter
• Foreign equivalency education evaluation if applicable
• Education verification from primary source (no hard copies accepted)
• Transcript/Diploma if education verification not on file

Health and Immunization Requirements (must be completed within 5 days of contract signing)

• Physical exam (within 1 year, updated annually)
• Fit mask test (within 1 year, updated annually)
• Negative TB test or chest X-ray (within 1 year)
• TB Questionnaire (in lieu of annual TB test renewal)
• Measles (Rubeola) immunity: positive titer or 2 vaccines (age exemption if born before 1957)
• Mumps immunity: positive titer or 2 vaccines (age exemption if born before 1957)
• Rubella immunity: positive titer or 1 MMR/Rubella vaccine (age exemption if born before 1957)
• Hepatitis B immunity or declination (special requirements for dialysis)
• Varicella immunity: positive titer, 2 vaccines, or documented history
• Drug screening (within 30 days of start)
• Seasonal flu vaccination or signed declination (required during flu season)
• Eye exam (Snellen, within 1 year)
• COVID vaccination copy preferred but not required

Competency Requirements

• Medication Test/Pharmacology Competency Exam (as required)
• Color Blind Testing (required once)
