Health & Immunization Requirements
- Physical exam within 1 year of start (must be completed within 72 hours of signing contract)
- Negative TB test or chest X-ray within 1 year of start (PPD or QGold/Tspot accepted)
- Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (positive titer, series, or declination accepted)
- Varicella
- Tetanus/Diphtheria/Tdap vaccine within 10 years of start
- Seasonal flu vaccine required during flu season (Oct 1 – Mar 31), with religious or medical exemptions accepted
- Fit mask (3M 1860 Regular or 1860S) required and must be updated annually
- Negative 10-panel drug screen with THC within 10 business days prior to start (no exceptions)
Competency & Testing
- Medication/Pharmacology competency exam as applicable
- Color blindness testing (online Ishihara test accepted)
- Pass/fail testing for telemetry nurses (if applicable)
- Skills checklist required and updated annually
