OT Home Health - Travel/Contract

Location: San Ramon, California
Contract Length: 13 weeks


Job Details

  • Specialty: Therapy/Rehabilitation (Occupational Therapy)
  • Setting: Home Health
  • Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days per week
  • Weekly Hours: 40 hours (guaranteed 36 hours weekly after initial ramp-up)
  • Patient Load: 5-6 patients per day, 1:1 ratio
  • Start Date: ASAP (compliance process takes about 2 weeks)
  • Weekend Work: Optional to meet productivity goals
  • On-Call: Not required
  • Locals Accepted: Yes
  • Travel Radius: Occasional travel up to 50+ miles; own transportation required
  • Documentation: Timely communication and documentation required; tools provided and must be returned
  • Attire: Any color scrubs
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Required Qualifications

  • Valid Occupational Therapist license in California
  • Minimum 1 year of Occupational Therapist experience within the last year
  • Experience in a Home Health setting
  • Current Basic Life Support (BLS) certification
  • Must have reliable personal transportation

  • Government-issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form
  • I9 Attestation
  • Background Checks including 7-year history and annual updates (SS Trace, County, Nationwide, Sex Offender, EPLS/GSA/SAM, HHS/OIG)
  • Certifications: BLS (required), ACLS and PALS preferred but not mandatory
  • State License verification (primary source online verification)
  • Competency exams as applicable (Medication/Pharmacology)
  • Personal or Auto Insurance (must be current throughout assignment)
  • Auto Registration (must be current throughout assignment)
  • Physical Exam within 1 year, cleared to work without restrictions
  • Immunizations and Titers:
    • Tdap within 10 years or annual declination
    • Negative TB test or chest X-ray within 1 year
    • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
    • Hepatitis B (titer, vaccine series, or declination)
    • Varicella (2 vaccines or positive titer)
  • Drug Testing: 10-panel drug screen within 1 year
  • Flu Vaccine or Declination required year-round
  • COVID-19 Vaccine documentation or declination form required

Experience Notes

  • First-time travelers are NOT accepted for this assignment
  • Prior travel experience is not explicitly required, but first-timers are not accepted

References & Verification

  • 2 professional references required (travel, allied, or per diem acute care) within 3 years
  • 1 year of employment verification related to OT profession
