- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Contract Length: 13 weeks
- Typical Caseload: 1:1 patient ratio; may visit up to 6 patients per day; 30 productivity points standard

Required Qualifications
- Valid Occupational Therapist license in Oregon
- Minimum 1 year of Occupational Therapist experience within the last year
- Experience in a Home Health setting
- Experience with OASIS documentation
- Basic Life Support (BLS) certification
Work Environment & Facility Information
- Facility Type: Home Health
- Attire: Business casual; scrubs allowed but not required; no jeans
- Charting Systems: EPIC Remote Client (laptop), Rover (phone app), and OASIS
- Parking: Free
- Mileage Reimbursement: IRS standard rates
Compliance & Background Requirements
- Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
- Drug screening: 9-panel drug screen within 12 months of start (instant acceptable)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
- Immunizations and health clearances required:
- Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
- Negative TB test or chest x-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Varicella vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Seasonal flu vaccine during flu season (declination accepted)
- Respiratory safety clearance required annually for travel candidates
- Fit mask testing required for travel candidates (updated annually)
- References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment application covering at least last 5 years required
Additional Notes
- Facility may call off up to one shift per pay period (every two weeks)
- No weekend or on-call shifts
- No floating between units
- No accommodation for block schedules (e.g., 4x10 or 3x12 shifts)
- Return-to-office (RTO) restrictions to be confirmed with hiring manager
- Annual regulatory and specialty-specific training modules are not required for home health at this facility
