Job Opportunity: Hospice Registered Nurse

Location: Arbor Vitae, Wisconsin

Contract Length: 13 weeks

Job Details

• Specialty: Hospice Registered Nurse (RN)
• Shift: Day shift, Monday through Friday, 8:00 AM to 5:00 PM
• Hours: 40 hours per week guaranteed
• Patient Load: 5-7 visits per day
• Travel: Up to a 60-mile radius around Arbor Vitae / Northwoods
• Cases: Geriatric, end-of-life care
• EMR System: Experience required with HCHB (Homecare Homebase)
• Dress Code: Scrubs of any color; jeans are not allowed
• Device Provided: iPad
• Cell Phone: Not allowed for work use
• Mileage: Reimbursed at IRS standard rate
• Weekend Work: Included

Required Qualifications

• Valid RN license in Wisconsin
• Minimum 1 year of hospice experience within the last year
• BLS certification issued by American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience with Homecare Homebase (HCHB) EMR system

Background and Compliance Requirements

• Background checks must be completed within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific searches as applicable (e.g., Wisconsin DOJ Criminal Search)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting may be required depending on state placement
• Education verification of highest degree related to profession
• NURSYS verification for RN license within 90 days of start
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Medication/Pharmacology competency exams as applicable

Health and Immunization Requirements

• Physical exam within the last 12 months (not required annually)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire required
• Immunizations or proof of immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months of start (instant acceptable)
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Additional Requirements

• Proof of current auto insurance required if job duties include driving
• Completion of facility-specific Acceptable Use Policy prior to start
• Two professional references within 2 years of start (travel, allied, per diem acute care)
• Full 7 years of work history documented with no gaps, verified within 90 days of start
• Annual skills checklist completion

Notes

• Locals are accepted and paid at travel rates
• On-call duties are not required
• First-time hospice travelers are not accepted for this position

This position is ideal for experienced hospice RNs who have prior travel experience and are comfortable managing a flexible territory with a focus on geriatric end-of-life care.
