Job Opportunity: COTA Home Health

Location

Concord, California
Travel radius up to 60 miles from the hub/office

Employment Type

Travel/Contract
Contract length: 13 weeks

Schedule

Monday to Friday, no weekends
Day shift: 8:00 AM – 5:00 PM (5 days x 8 hours = 40 hours/week)
On-call required every 3rd to 4th weekend

Patient Population & Caseload

Geriatric cases
Average 5-7 patients per day

Required Qualifications

Valid Certified Occupational Therapy Assistant license in California
Minimum 1 year of recent Certified Occupational Therapy Assistant experience
Basic Life Support (BLS) certification

Additional Requirements

Must meet productivity points to qualify for guaranteed hours
Dress code: Scrubs of any color; no jeans allowed
No personal cell phone use; iPad provided
Mileage reimbursed at IRS standard rate
Locals accepted at travel rate
First-time travelers accepted

Compliance & Documentation

Government-issued photo ID (Driver’s License/State ID)
Emergency contact form required prior to start
Background checks within 30 days and annually, including federal, county, sex offender, FACIS Level 3, and motor vehicle record
Education verification required
Physical exam within 6 months prior to start
Negative TB test or chest X-ray within 6 months (with specific rules for past positive TB)
Immunizations or proof of immunity for Measles, Mumps, Rubella, Hepatitis B, Varicella
9 or 10 panel drug screen within 30 days to 1 year of start
Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
Work history verification for last 3 employers
Two professional references required for travel, allied, or per diem acute care roles
Annual skills checklist and competency exams as applicable
