Job Opportunity: Registered Nurse (Cath Lab)

Location: Findlay, Ohio
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months)

Job Description

• Position: Cath Lab RN
• Experience with Cath Lab required
• On-call requirement: every 4th weekend
• Experience with Phillips Azurion and Merge Hemo for documentation is a bonus

Required Credentials & Documentation

Licensure & Certifications

• Valid State Nursing License (Primary Source Verification required)
• ACLS (Advanced Cardiac Life Support) - as applicable
• BLS (Basic Life Support) - as applicable
• PALS (Pediatric Advanced Life Support) - as applicable
• NRP (Neonatal Resuscitation Procedure) - as required
• TNCC (Trauma Nursing Core Course) - as required
• NURSYS Verification (for RNs)
• Orientation Safety Manual Acknowledgement (HIPAA compliance)

Background Checks

• 7-year criminal background check within 30 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM check within 30 days of start and updated for contract renewals
• HHS/OIG check within 30 days of start and updated for contract renewals
• Education verification

Health & Immunization Requirements

• Physical exam attesting ability to meet job demands (initial and annually)
• Negative TB test (PPD, Quantiferon Gold, or T-spot) or chest X-ray within 1 year of start; annual TB questionnaire required
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (chickenpox)
• Seasonal flu vaccination or signed declination (flu season Oct 1 – Mar 31; must be compliant by Dec 1 to work)
• COVID-19 vaccination required (fully vaccinated or approved medical/religious exemption with accommodations)
• Drug screening: Negative 10-panel drug screen within 30 days of start

Competency Exams

• Medication Test/Pharmacology Competency Exam (as applicable)
• Unit Competency Exam (updated annually)

Additional Documentation

• Proof of I9 or attestation
• Government-issued photo ID carried during work hours
• Residency Declaration Form if working on a compact license
• Professional references (2 clinical references or hospital evaluations)
• Employment application/resume
• Skills checklist (updated annually)
• Contract employee checklist, orientation acknowledgment, badge authorization, confidentiality agreement, system access form, and other facility-specific forms must be completed and signed

Work Schedule & Conditions

• On-call every 4th weekend
• Must comply with all facility-specific policies and documentation requirements
• Safety accommodations for COVID-19 exemptions include masking, possible PCR testing, and social distancing
