Job Opportunity: Occupational Therapist (Travel/Contract)

Hospital System: Renaissance Emergency Services - Locums

Job Details

  • ASAP start
  • 8-hour shifts
  • Per diem position with a minimum of 4 weekend shifts per month
  • Additional hours available based on availability
  • Ability to float to other UCSF facilities

Requirements

  • Valid California OT License
  • AHA Basic Life Support (BLS) certification
  • Master’s degree (MS/MA)
  • Minimum 1 year of inpatient rehab experience
  • Must have IRF-PAI (Inpatient Rehabilitation Facility-Patient Assessment Instrument) experience

  • State License Verification (primary source)
  • NURSYS Verification (for RN, LPN/LVN, AP)

Competencies

  • Medication Test/Pharmacology Competency Exam (as applicable)

Health & Immunization Requirements

  • Physical exam within 1 year of start
  • Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest x-ray within 1 year
  • Rubeola (Measles), Mumps, Rubella: Vaccine or positive titer documentation as applicable (especially for Mother-Baby Units)
  • Hepatitis B: Series, titer, or declination
  • Varicella: Not required unless specified by client
  • 9 Panel Drug Screen within 1 year of start
  • Medical and Employee Information Release Form

Background & History Checks

  • Background check within 1 year of start (7-year search, including SSN trace, county, nationwide, sex offender searches)
  • EPLS/GSA/SAM check
  • HHS/OIG check
  • References:
    • 2 professional references for travel, allied, per diem acute care
    • 1 professional reference for per diem non-acute care
  • Skills checklist (required annually)
