Job Description

• Provide physical therapy assistant services in a home health setting.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
• Must be willing to travel up to 75 miles from the home clinic.
• Mileage reimbursed at IRS standard rate (patient home to patient home).
• Use of EMR systems: HomeCare HomeBase, Oasis, Outlook.
• No on-call required for this position.
• Local candidates accepted.

Required Qualifications

• Valid Physical Therapy Assistant license in Georgia.
• Minimum of 1 year of recent Physical Therapy Assistant experience within the last year.
• Experience working with adult patients in a home health setting.
• BLS certification.

Additional Information

• Weekend requirements vary; discuss during interview.
• Some positions may have alternate shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8s Monday-Friday.
• Contractor will be assigned 5-10 patients per day, equating to roughly 6-8 points per day (points relate to visit types and activities).
• No guarantee of hours; if patient census is low, floating to a nearby clinic may be discussed.

Compliance and Documentation Requirements

• Valid driver's license/state ID.
• Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and fingerprinting as required by state.
• Education verification.
• Various state-specific background and registry checks depending on assignment location.
• Health documents including:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Drug screening (10 panel).
  • Seasonal flu vaccination or declination.
  • COVID vaccination or declination.
• Completion of compliance training, confidentiality agreements, privacy policies, and other facility-specific documents prior to start.
• Personal or auto insurance policy required for home health placements (policy must be current and under candidate’s name or list candidate as a driver).

Travel Experience

• No explicit prior travel experience required.

References and Work History

• Two professional references required.
• Verification of at least two most recent employers or last 7 years of work history.
