Required Qualifications

• Valid Physical Therapist license in Illinois
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS
• Experience in a(n) Home Health setting
• Driver's License

Preferred Qualifications

• OASIS and start of care experience

Role Details

• Evaluates patient needs, establishes goals, and develops and implements individualized physical therapy plans
• Provides age-appropriate care from pediatric to geriatric patients
• Assesses, plans, treats, evaluates, and implements a plan of care
• Communicates with patients, families, physicians, and staff
• Completes documentation in the electronic medical record
• Transfers and positions patients
