Home Health Registered Nurse

Overview

• Guaranteed hours: 38 per week, with flex to 40
• Orientation: 4 days

Required Qualifications

• Valid RN license in Wisconsin
• Associate of Science in Nursing / Associate Degree in Nursing
• Home Health (2 years of experience)
• Home Health experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or American Red Cross
• Driver's License
• Experience with Epic
• Experience with OASIS

Preferred Qualifications

• Case management experience
• OASIS certification

Assignment Details

• Seeing patients in the Stevens Point and Plover area
• May float to adjacent territories based on census; no long-distance or non-adjacent floating
• Expected to assist in case managing Stevens Point/Plover area patients as needed
• Every 3rd weekend and every third holiday
• On-call 5 times per month
• Expected to see 4–6 patients per day
• Productivity benchmark: 1.95 hours per visit
• Free onsite parking
• Any scrub color

Clinical Responsibilities

• OASIS admissions and time points
• Episode management
• Doctor calls and coordination of care with PT/OT and other disciplines
• Wound care and wound vacs
• IV therapy, including PICC lines and ports for IV antibiotics
• Lab draws
• Enteral feeding: G-tubes and J-tubes
• Urinary catheter care: Foley and suprapubic
• Ostomy care, including new ostomies
• Chronic disease management, including diabetes, HF, and CAD

On-Call Expectations

• Must respond to answering service calls within about 15 minutes
• On-call is primarily triage, with additional visits as needed for issues such as blocked Foley or wound vac malfunction
• If called back to the facility, call-back minimum is 2 hours

Additional Notes

• Contract is eligible for 36 hours of unbillable low census hours
• Travelers may be placed on call for low census hours, and those hours count toward the 36 low census unbillable hours
• During flu season, travelers who decline flu and COVID vaccination must wear a mask at all times
