May require coverage in other territories within the facility’s service area, up to about a 60-mile radius from the facility address

Required Qualifications

Valid Physical Therapist license in Pennsylvania
Experience in a Home Health setting
Homecare Homebase documentation experience
Experience with OASIS

Assignment Details

Average 4–6 visits per day, depending on visit type
Productivity expectation: 28–32 points per week
Overtime must be pre-approved in writing by the administrator
2-week cancellation notice

Notes

The client may consider candidates without Homecare Homebase experience at a reduced rate for the first 2 weeks if the manager is willing to train
